Individual sessions are 50 minutes.
Group sessions are 90 minutes.
Insurance
Euronda Travis, LPC is NOT in-network with insurance companies. I am an out-of-network provider. Some insurance companies will reimburse you for out-of-network expenses. At your request, we will provide a monthly statement to submit to your insurance company for reimbursement.
HOW TO CHECK YOUR OUT-OF-NETWORK COVERAGE
- Plan for the call to last 15-30 minutes.
- You will need the following:
- Insurance card
- Name, date of birth, and social security number
- Pen and paper
- Call the number on the back of your insurance card.
- Ask the following questions:
- Does my policy include outpatient mental health benefits?
- If yes, does my policy cover Telehealth (online) sessions?
- Does my policy include out-of-network benefits?
- What are the requirements to use out-of-network benefits? Do I have an out-of-network deductible?
- If yes:
- What is the deductible?
- How much has been met?
- If yes:
- Is a prior authorization required?
- Is a referral needed from a primary care physician?
- What percentage will be reimbursed to me?
- How do I submit a claim for reimbursement?
- How long do I have to submit claims?
- Does my policy include outpatient mental health benefits?
